The Community Paramedicine (CPM) course introduces EMS professionals to an expanded model of care that supports patients beyond the traditional emergency response system. The program focuses on helping providers deliver preventive, follow-up, and in-home services that improve patient outcomes and reduce unnecessary hospital utilization.
Key areas of instruction include:

  • Conducting in-home assessments and identifying risk factors

  • Coordinating with primary care, social services, and community resources

  • Managing chronic conditions, medication compliance, and patient education

  • Supporting vulnerable populations through proactive, non-emergency care

CPM prepares paramedics and EMS personnel to play a vital role in community health by providing patient-centered care where it is needed most.
